The head — measured from the tip of the chin to the top of the head, hair included — must fill between 50% and 69% of the photo height. On a 45 mm tall UK photo, that's a chin-to-crown height between roughly 22.5 mm and 31 mm; HMPO tightens the range to 29–34 mm for its own enforcement.

Eye position is a vertical rule enforced numerically by the US and by digital portals in several other countries. On a 2x2 inch US passport photo, the eyes must sit between 1 1/8 and 1 3/8 inches from the bottom edge. In practical terms, that puts the eyes just above the vertical center — closer to the top than to the bottom.
The rule exists so that facial recognition algorithms can normalize the face to a standard template. The algorithm expects the eyes at a specific height; if they're too high (head tilted back) or too low (head tilted down), the geometry doesn't match and the confidence score drops.
For countries that don't publish a specific eye-height number (UK, Canada), the rule is inferred from the combination of head height and vertical centering: if the head is the correct size and roughly centered vertically with a small margin above, the eyes land in the correct zone by default.
